CNS Medical Group

Craig Hospital has eight on-site physicians who belong to the CNS Medical Group P.C. and whose full-time practice is in spinal cord and brain injury rehabilitation. They average 17 years at Craig and are among the world leaders in the field. With offices in the hospital, our physicians are readily accessible to patients, families, and staff. Each patient at Craig has a primary physician, and a team of therapists and nurses that work with each physician. The CNS physicians direct the clinical interdisciplinary treatment teams and are supported by 140 consulting physicians representing the full spectrum of medical specialties. The CNS Medical Group consists of:

Daniel P. Lammertse, M.D., Medical Director

Daniel P. Lammertse, M.D., Medical Director
Dr. Lammertse came to Craig Hospital in 1981 and has served as Medical Director since 1984. He received his medical degree and specialty training in rehabilitation medicine at Ohio State University. He is board certified in Physical Medicine and Rehabilitation and has a sub-specialty certification in spinal cord injury. He has authored numerous chapters and scientific articles on topics in spinal cord injury and is internationally recognized as an expert in spinal cord injury rehabilitation. He is the project director of the Rocky Mountain Regional Spinal Cord Injury System and served as Chair of the NIDRR SCI Model Systems Project Directors Committee from 2000-2006. Dr. Lammertse served on the Board of Directors of the American Spinal Injury Association from 1991-2003 and was President of the association from 2001-2003. He is on the Editorial Board of the Journal of Spinal Cord Medicine and is a member of the Spinal Cord Injury Medicine Examination Committee of the American Board of Physical Medicine and Rehabilitation. Dr. Lammertse has an appointment as Associate Clinical Professor of Rehabilitation Medicine at the University of Colorado.

Thomas E. Balazy, M.D.

Dr. Balazy supervises one of Craig's spinal cord injury teams. Dr. Balazy has been at Craig since 1987 and for 17 years Dr. Balazy was Craig's Medical Director at the Mulitrauma unit at Swedish Medical Center. Dr. Balazy received his medical degree and completed his residency at the University of Colorado Health Sciences Center, and completed a fellowship in spinal cord and brain injury at Craig. He is also assistant clinical professor at the University of Colorado Health Sciences Center. Dr. Balazy is board certified in Physical Medicine and Rehabilitation. In 2006 Dr. Balazy received the Jerome Gersten Clinical Teaching Award from the Dept. of PM&R at the UCHSC School of Medicine. Dr. Balazy is the author of a widely read publication on chronic pain in spinal cord injury.

Shih-Fong (Mike) Hsu, M.D.

Dr. Hsu received his medical degree and completed his residency at the National Taiwan University Hospital. He completed his specialty training in rehabilitation medicine at the University of Colorado Health Sciences Center and went on to fellowship training in spinal cord injury and brain injury rehabilitation at Craig Hospital. He is board certified in Physical Medicine and Rehabilitation. Dr. Hsu served as Medical Director of the Neurotrauma Unit at Swedish Hospital between 1984 and 1986. He is an assistant clinical professor at the University of Colorado Health Sciences Center. Dr. Hsu has extensively researched tracheal and esophageal complications in spinal cord injury. Dr. Hsu's spinal cord injury team recently was chosen to pilot the development of SCI Critical Pathways for the hospital.

Mark R. Johansen, M.D.

Dr. Johansen supervises one of Craig's spinal cord injury rehabilitation teams. Dr. Johansen joined the CNS Medical Group in July 2001 following a subspecialty Fellowship program in neurotrauma rehabilitation at Craig. He received his medical degree and completed his residency training in rehabilitation medicine at the University of Utah where he served as chief resident and was honored with the Mark P. Scott Memorial Award for outstanding resident. Dr. Johansen finished his year of internship in internal medicine at St. Joseph Hospital in Denver. In 2004 Dr Johansen received a sub-specialty certification in SCI by the Board of the American Academy of Physical Medicine and Rehabilitation.

Indira S. Lanig, M.D.

Dr. Lanig has been at Craig Hospital since 1989 and directs one of the spinal cord injury teams. She received her medical degree and completed her residency at Baylor College of Medicine. She is board certified in Physical Medicine and Rehabilitation. Dr. Lanig currently is an assistant clinical professor at the University of Colorado Health Sciences Center in the department of physical medicine and rehabilitation. She has authored several chapters on topics in spinal cord injury and was lead author with other Craig staff on a textbook on Health Promotion and SCI. Dr. Lanig is the current President of the American Paraplegia Society (APS), a national SCI scientific association of approximately 450 clinicians, neuroscientists, and researchers. She has served on the Scientific Advisory Board of the Paralyzed Veterans of America Spinal Cord Research Foundation. Dr. Lanig has a sub-specialty certification in SCI by the Board of the American Academy of Physical Medicine and Rehabilitation. Dr. Lanig was the recipient of the 2005 Baylor College of Medicine/University of Texas Health Science Center-Houston Physical Medicine and Rehabilitation Alliance Distinguished Alumnus Award.

Gary A. Maerz, M.D., M.B.A.

Dr. Maerz joined the CNS Medical Group in July 2001 under a new combined neurotrauma rehabilitation fellowship/junior-attending program. He received his medical degree from the University of Colorado Health Sciences Center. Subsequent to an internship in general surgery at St. Joseph Hospital in Denver, Dr. Maerz completed residency training as Chief Resident in Physical Medicine and Rehabilitation at the University of Colorado. Prior to his medical career, Dr. Maerz pursued undergraduate and graduate education and work experience in engineering and business administration. He is a licensed Professional Engineer with special interest in the application of assistive technology for persons with brain and spinal cord injury.

David L. Ripley, M.D.

David L. Ripley, M.D.
David L. Ripley, MD began his practice with the CNS Medical Group at Craig Hospital in 2004 and directs one of Craig's traumatic brain injury treatment teams. Dr. Ripley received his degree from the Medical College of Georgia. He performed his internship at the Medical College of Georgia and he performed his residency at the Medical College of Virginia Hospital, where he served as the Chief Resident during his last year of residency. Dr. Ripley comes to Craig Hospital after serving most recently as the Medical Director of Post Acute Services and the Medical Director of Brain Injury Research at The Shepherd Center in Atlanta, GA. He is board certified in Physical Medicine and Rehabilitation and is a Fellow of the American Academy of Physical Medicine and Rehab. Prior to medical school, he worked as a Certified Vocational Rehabilitation Counselor and retains certification as a CRC.

Alan H. Weintraub, M.D.

Dr. Weintraub is the Medical Director of the Brain Injury Treatment Team and Medical Director of the TBI Model Systems Grant Program. Dr. Weintraub has been at Craig since 1985 after he completed his residency at University of Colorado Health Sciences Center and his fellowship at Craig. He is board certified in Physical Medicine and Rehabilitation and is an assistant clinical professor at the University of Colorado Health Sciences Center. He was a board member for the Rocky Mountain Regional Brain Injury Center and is a member of the Colorado Division of Workers Compensation Task Force for the development of Medical Treatment Guidelines for TBI. Dr. Weintraub's interests include pharmacologic rehabilitation and sports-related brain injury, and also is currently co-developing a Spasticity Clinic.

Green bulletHow You and Our Physicians Can Work Together

If you are a physician wanting to refer a new patient to Craig Hospital...
If you are a physician anywhere in the U.S. and you want to refer a new patient to Craig for inpatient rehabilitation or outpatient evaluation, ask your discharge planner or office staff to call our Admissions Department directly. If you would like to discuss your referral with one of our Craig doctors, we encourage you to call our CNS Medical Group and ask for the On-Call Referral physician. One of our doctors will speak with you personally, take the referral information, discuss the patient's condition, and facilitate the referral process with our Admissions Department. We recommend that the referral be called in as soon as possible to expedite admission. If you would like to receive physician reports from Craig during the patient's treatment at Craig, please obtain the necessary authorization from the patient and give your mailing address to the CNS Medical Group or our Admissions Department. Request that your information appear on the patient's Craig "Face Sheet."

If you are a physician treating a former Craig patient...
If you are a doctor and you want to discuss your patient with his/her Craig doctor, or you want to refer a former Craig patient for a re-evaluation, rehabilitative neurosurgical consultation, skin evaluation, etc., please call our CNS Medical Group and ask for the patient's former Craig physician. Your call is not mandatory, but encouraged. For re-evaluation scheduling, former patients have been told to contact our Admissions Department directly, but you are encouraged to call as well. Craig offers a 4-5 day comprehensive re-evaluation program for patients with spinal cord injury, and other individually-tailored evaluations, re-evaluations, or clinic services. If you would like to receive physician reports from Craig during the patient's treatment at Craig, please obtain the necessary authorization from the patient and give your mailing address to the CNS Medical Group or our Admissions Department. Request that your information appear on the patient's Craig "Face Sheet."

If you are a physician needing clinical information about SCI or TBI...
If you need any clinical information about SCI or TBI, feel free to call our CNS Medical Group. One of us will take your call, or call you back if we are not immediately available. Depending on the nature of your questions, we may also refer you to others at Craig who have specific expertise Ñ nurses, research professionals, therapists, etc. If you need any brochures or videotapes describing Craig's programs for your office or patients, Provider Relations can help. We invite you to utilize Craig as a resource for patients with SCI and TBI, regardless of whether we have a patient in common. We cannot diagnose or recommend treatment for patients for whom we haven't seen, but we can offer helpful guidance and direction on clinical issues or concerns.
